ABSTRACT

Fundamentals: Atopic Dermatitis (AD) and Psoriasis (PS) share clinical and physiopathological similarities. Objective: Determine the prevalence of sensitization to Malassezia spp. in adults with AD and PS and its correlation with disease severity. Methods: A cross-sectional study was carried out from January 2016 to August 2017 with adults. Malassezia spp.-specific IgE dosages were measured, and skin scrapings for fungal culture performed. Parametric or nonparametric tests were used for analysis. Results: Median age of the 20 participants with AD was 29 years old, and the mean SCO-RAD was 45.35 ± 18.32. Malassezia spp.- specific IgE median dosage was 0.63 kU/l. M. furfur and M. sympodialis were isolated. Spearman's nonparametric correlation analysis showed no correlation between sensitization to Malassezia spp. and disease severity. The median age of the 36 participants with PS was 61 years old, the median body surface area affected was 22%, and Malassezia spp.-specific IgE median dosage was 0.00 kU/l. M. furfur and Malassezia spp. were identified. Study limitations: Assessing the sensitization to Malasseziaspp. was difficult due to the reduced number of participants in the study. Furthermore, there was no uniformity in the location to collect skin scrapings. The use of topical medication was not suspended before collecting skin specimens for mycological examination, therefore interfer-ing with fungal isolation. Conclusion: Sensitization to Malassezia spp. was only detected in the AD sample. Malassezia spp.-specific IgE test did not prove to be a marker for disease severity in our AD sample (AU)


Fundamentos: Dermatite atópica (DA) e psoríase apresentam similaridades clínicas e fisiopatológicas. Objetivos: Avaliar a frequência da sensibilização a Malasseziaspp. em adultos portadores de DA e psoríase e correlacionar à gra-vidade dos quadros clínicos. Métodos: De janeiro de 2016 a agosto de 2017, conduziu-se um estudo observacional em indivíduos adultos onde foram realizadas dosagem de IgE específica anti-Malassezia spp. e raspados das lesões para cultura micológica. Testes paramétricos ou não paramétricos foram utilizados para análise. Resultados: Nos 20 portadores de DA, a mediana da idade foi 29 anos. O valor médio do Scoring Atopic Dermatitis foi 45,35 ± 18,32. A mediana de IgE específica anti-Malasseziaspp. foi 0,63 kU/l. M. furfur e M. sympodialis foram isolados. A análise de correlação não-paramétrica de Spearman não mostrou correlação entre a sensibilização à Malassezia spp. e a gra-vidade. Nos 36 pacientes com psoríase, foram obtidas as seguintes medianas: idade 61 anos, comprometimento de superfície corpórea 22% e IgE específica anti-Malassezia spp. 0,00 kU/l. Houve identificação de M. furfur e Malasse-zia spp. Limitações do estudo: O número reduzido de participantes dificultou a avaliação da sensibilização por IgE a Malasseziaspp. Não houve uniformidade nos locais de coleta dos raspados cutâneos. Medicamentos tópicos não foram suspensos anteriormente ao exame micológico, prejudicando o isolamento dos fungos. Conclusões: Sensibili-zação a Malassezia spp. apenas ocorreu nos portadores de DA. O teste de IgE específica anti-Malassezia spp. não se mostrou um marcador de gravidade para a DA neste grupo (AU)

ABSTRACT

Atopic dermatitis (AD) is highly comorbid with negative emotions such as anxiety and depression. Although acupuncture has demonstrated efficacy in AD, its influence on comorbid anxiety and depression remains unclear. We sought to explore the impact and mechanisms of action of acupuncture on comorbid anxiety and depression of AD. AD-like skin lesions were induced by the topical application of MC903 to the mouse cheek. Acupuncture was performed at Gok-Ji (LI11) acupoints. AD-like phenotypes were quantified by lesion scores, scratching behavior, and histopathological changes. The effects of acupuncture on comorbid anxiety and depression-like behaviors were assessed using the elevated plus-maze (EPM), open-field tests (OFT), and tail-suspension test (TST). In addition, biochemical changes in the brain reward regions were investigated by immunoblotting for the expression of tyrosine hydroxylase (TH), dopamine D1 receptor (D1R), phospho-dopamine and cAMP-regulated phosphoprotein-32 kDa (pDARPP-32), phospho-cAMP response element binding protein (pCREB), ΔFosB, and brain-derived neurotrophic factor (BDNF) in the nucleus accumbens, dorsolateral striatum, and ventral tegmental area. Acupuncture effectively improved the chronic itching and robust AD-like skin lesions with epidermal thickening. Additionally, it considerably reduced comorbid anxiety- and depression-like symptoms, as indicated by more time spent in the open arms of the EPM and in the center of the open field and less time spent immobile in the TST. Higher pCREB, ΔFosB, BDNF, and pDARPP-32 levels, and reduced TH and D1R protein expression in the brain reward regions of AD mice were reversed by acupuncture treatment. The beneficial effects of acupuncture on clinical symptoms (scratching behavior) and comorbid psychological distress in AD strongly correlated with dorsal striatal ΔFosB levels. Collectively, these data indicate that acupuncture had a significant, positive impact on comorbid anxiety- and depression-like behaviors by modulating neuroadaptation in the brain reward circuit in mice with AD, providing a novel perspective for the non-pharmacological management of psychiatric comorbidities of AD.

Introdução: A dermatite atópica é uma doença crônica e recidivante, influenciada por fatores ambientais, que necessita prescrições médicas frequentes. O objetivo deste estudo foi investigar se crianças e adolescentes com dermatite atópica atendidos em um serviço público tiveram agravamento das lesões ou dificuldades no autocuidado durante a pandemia COVID-19.Métodos: Estudo longitudinal retrospectivo. Os dados foram coletados do prontuário de pacientes com dermatite atópica menores de 18 anos atendidos em um ambulatório público de referência do Rio Grande do Sul. A percepção sobre o agravamento das lesões foi avaliada pelos testes Qui-quadrado ou Exato de Fisher e regressão de Poisson foi utilizada para estimar o risco de agravamento das lesões conforme o medo da COVID-19.Resultados: Foram avaliados 33 pacientes, sendo 70% do sexo feminino e 57,6% residentes na capital. A média de idade foi de 9,6 ± 3,9 anos e o tempo médio de acompanhamento pelo serviço 4,6 ± 2,9 anos. A percepção de agravamento das lesões durante o período de suspensão dos atendimentos esteve presente em 81,8% das crianças/adolescentes. As crianças relataram com frequência o aumento de coceira (78,3%) e descamação (60,9%), enquanto os adolescentes referiram maior aumento da vermelhidão (60%). O medo de contrair o vírus foi associado a um risco oito vezes maior de apresentar agravamento das lesões.Conclusões: O alto percentual de crianças/adolescentes com agravamento das lesões e a associação da piora da doença com o medo relacionado à pandemia reforçam a importância do acompanhamento destes por equipe multidisciplinar que observe fatores fisiopatológicos e psicossociais.


Introduction: Atopic dermatitis is a chronic and recurrent condition, influenced by environmental factors, that requires frequent medical prescriptions. The aim of this study was to investigate if children and adolescents with atopic dermatitis treated at a public clinic reported exacerbation of the disease or self-care problems during the COVID-19 pandemic.Methods: We conducted a retrospective longitudinal study. Data were collected from the medical records of patients aged < 18 years with atopic dermatitis treated at a public outpatient clinic in Rio Grande do Sul, Brazil. The perception of disease exacerbation in children and adolescents was evaluated using the chi-square test or Fisher's exact test. Poisson regression was used to estimate the risk of worsening of skin lesions according to the fear of COVID-19. Results: The study sample included 33 patients, of whom 70% were female and 57.6% lived in the state's capital city. Mean patient age was 9.6 ± 3.9 years, and mean time of clinic follow-up was 4.6 ± 2.9 years. The perception of disease exacerbation during the pandemic was reported by 81.8% of children/adolescents. Children reported increased itching (78.3%) and desquamation (60.9%), whereas adolescents reported increased redness (60%). The fear of contracting COVID-19 was associated with an eight-fold increased risk of worsening of skin lesions.Conclusion: The high percentage of children/adolescents with worsening of skin lesions and the association between disease exacerbation and the fear of COVID-19 reinforce the importance of a multidisciplinary approach that focuses on pathophysiological and psychosocial factors in patients with atopic dermatitis.

INTRODUCCIÓN: La eritrodermia es un síndrome inflamatorio cutáneo infrecuente caracterizado por compromiso eritematoso generalizado y descamación, de más del 90% de superficie cutánea total. OBJETIVO: Caracterizar clínica e histopatológicamente a los pacientes con eritrodermia en un hospital universitario chileno. METODOLOGÍA: Estudio retrospectivo, realizado en el Hospital Clínico Universidad de Chile, basado en revisión de fichas clínicas e informes histopatológicos de pacientes con eritrodermia, entre 2005 y 2018. Se evaluó edad, sexo y variables clínicas (co-morbilidades, síntomas, días de evolución, ingreso hospitalario, informe histopatológico, diagnóstico y evolución). RESULTADOS: Total de 28 pacientes, 18 hombres (64%), edad promedio 59 años. Causa más frecuente de eritrodermia fue dermatosis pre-exis-tentes, con 15 casos (54%), que incluyen: psoriasis 9 (32%), dermatitis de contacto 3 (11%), PRP 2 (7%), dermatitis atópica 1 (4%). A estas le siguen: reacción adversa medicamentosa 6 (21%), idiopática 6 (21%) y Síndrome de Sezary 1 (4%). CONCLUSIÓN: El presente estudio corresponde a la primera serie de eritrodermias realizada en Chile. Destacan las dermatosis preexistentes como la principal causa, lo que se correlaciona con la literatura.


INTRODUCTION: Erythroderma is an infrequent cutaneous inflammatory disorder characterized by generalized erythematous compromise and desquamation, of more than 90% of total cutaneous surface. OBJECTIVE: Clinical and histopathological cha-racterization of patients with erythroderma in a Chilean university hospital. METHODOLOGY: Retrospective study, performed at the University of Chile Clinical Hospital, based on review of clinical records and histopatho-logical reports of patients with erythroderma, between 2005 and 2018. Age, sex and clinical variables were evaluated (co-morbidities, symp-toms, days of evolution, hospital admission, histopathological report, diagnosis and evolu-tion). RESULTS: A total of 28 patients, 18 were men (64%), average age 59 years. Most frequent cause of erythroderma was pre-existing dermatosis, with 13 cases (52%), which included: psoriasis 9 (32%), contact dermatitis 3 (11%), PRP 2 (7%), atopic dermatitis 1 (4%). These are followed by adverse drug eruption 6 (21%), idiopathic 6 (21%) and Sezary syndrome 1 (4%). CONCLUSION: The present study corresponds to the first series of erythrodermas performed in Chile. The pre-existing dermatoses were the main cause of erythroderma, which coincides with other reports.

RESUMO Objetivo: Realizar tradução, adaptação e validação da Itch Severity Scale (ISS-Ped) para medir a gravidade de prurido em crianças e adolescentes com dermatite atópica. Métodos: Estudo metodológico de validação de instrumento, seguindo protocolos recomendados. Após a definição da versão traduzida, com participação de um profissional com experiência em validação de instrumentos, três professores de inglês, um professor de linguística e sete alergologistas, a Escala de Gravidade de Prurido (ISS-Ped) foi aplicada a 42 responsáveis por pacientes entre 2 e 18 anos de idade com dermatite atópica e a 42 responsáveis por indivíduos sem doença cutânea pruriginosa da mesma faixa etária. Resultados da escala foram comparados com a gravidade da dermatite atópica e o controle da doença, e entre os dois grupos. Resultados: A clareza das questões foi maior que 90%. A ISS-Ped mostrou forte correlação positiva com a gravidade da dermatite atópica (Pearson: 0,74; p<0,001) e boa correlação com o controle da doença (coeficiente de correlação ponto bisserial: 0,65; p<0,001), além de ótima consistência interna (α de Cronbach: 0,96) e adequada reprodutibilidade pela concordância do teste e reteste (coeficiente de correlação intraclasse variando de 0,89 a 0,99 com IC95% e p<0,001). Conclusões: A ISS-Ped apresentou-se viável, válida e confiável, sendo atingida equivalência satisfatória. A escala traduzida mostrou-se adequada para avaliar a gravidade do prurido em crianças e adolescentes com dermatite atópica, permitindo comparações na prática clínica e entre pesquisas de diferentes centros.


ABSTRACT Objective: To translate, adapt and validate the Itch Severity Scale to a Brazilian version (ISS-Ped) in order to measure the severity of pruritus in children and adolescents with atopic dermatitis. Methods: This is a methodological study of validation of an instrument following recommended protocols. The translated version was evaluated by a group of experts including one professional with experience in instrument validation, three English teachers, one linguistics teacher and seven allergists. After this, the scale was applied to 42 parents of children aged between 2 and 18 years old with atopic dermatitis, and 42 parents of children without pruritic diseases. Results were evaluated according to the severity of atopic dermatitis and disease control, and they were compared between groups with and without atopic dermatitis. Results: More than 90% of the questions were clear to the parents. The ISS-Ped showed a strong positive correlation with the severity of atopic dermatitis (Pearson: 0.74; p<0.001) and a good correlation with the control of dermatitis (point-biserial correlation coefficient: 0.65; p<0.001). The scale showed excellent internal consistency (Cronbach's α: 0.96) and adequate test and retest agreement (95% confidence interval of intraclass correlation coefficient: 0.89-0.99; p<0.001). Conclusions: The ISS-Ped is a feasible, valid, reliable and satisfactorily equivalent. The translated scale was appropriate to assess the severity of itching in children and adolescents with eczema, allowing comparisons in the clinical practice and in the research setting.

This study evaluated the concentration of Der p 1, Der f 1 and Blo t 5 in the fur and households of 20 dogs with atopic dermatitis (AD) and 20 healthy dogs. The diagnosis of AD was clinical based on Favrot's criteria. Dust samples were collected with a domestic vacuum cleaner. For each site, 1m2 was vacuumed for 2 min. The samples were collected in separate filters, transferred into plastic containers, sealed and kept frozen until ELISA analysis. In the fur of atopic dogs the average concentration of Der p 1 was 0.25µg/g compared to 0.03µg/g in healthy dogs. In households with atopic dogs the highest concentrations of Der p 1 were found in carpets (2.18µg/g), followed by couches (1.53µg/g), beds (1.14µg/g), dogs' bed linen (0.64µg/g) and floors (0.14µg/g). The concentrations of Der p 1 on carpets, couches and beds were significantly higher than in atopic dogs' fur (p<0.05). There was no statistical difference when comparing the concentrations of Der f 1 and Blo t 5 in different environments of atopic dogs (p>0.05). The concentrations of Der p 1, Der f 1 and Blo t 5 were equivalent in atopic and non-atopic dog's households. Among the allergens studied, Der p 1 was the most commonly found, predominantly in carpets and couches.(AU)


O presente estudo avaliou a concentração de Der p 1, Der f 1 e Blo t 5, na pelagem e no ambiente domiciliar de cães com dermatite atópica (DA). Para tal, foram selecionados 20 cães com DA e 20 cães saudáveis, e seus domicílios. O diagnóstico de DA foi baseado nos critérios de Favrot. As amostras de poeira foram colhidas com um aspirador de pó doméstico. Para cada local de colheita, foi aspirado 1m2 por dois minutos. As amostras foram recolhidas em filtros separadamente, transferidas para envelopes plásticos, seladas e mantidas congeladas até serem analisadas pelo método de ELISA. Na pelagem dos cães com DA, a concentração média de Der p 1 foi de 0,25µg/g de poeira e nos cães saudáveis foi de 0,03µg/g. No ambiente de cães com DA, o Der p 1 foi encontrado em maior concentração média no tapete (2,18 µg/g), seguido pelo sofá (1,53g/g), cama (1,14µg/g) e roupa de cama dos cães (0,64µg/g) e chão (0,14µg/g). As concentrações de Der p 1 no tapete, sofá e cama dos cães foram significativamente maiores que na pelagem de cães com dermatite atópica (p<0,05). Enquanto que, não houve diferença estatística quando comparadas as concentrações de Der f 1 e Blo t 5 nos diferentes ambientes avaliados (p>0,05). As concentrações de alérgenos Der p 1, Der f 1 e Blo t 5 se equivaleram em ambientes de cães com DA e saudáveis. Entre os alérgenos estudados, o Der p 1 foi o mais comumente encontrado, prevalecendo no tapete e no sofá.(AU)

ABSTRACT

O desenvolvimento de novos modelos de pele e novas metodologias in vitro segue uma tendência mundial na busca pela redução ou substituição de testes em animais. Nesse contexto, kits de epiderme humana reconstruída (RHE) apresentam-se como uma plataforma promissoras para essa proposta e, alguns modelos encontram-se validados para ensaios de irritação e corrosão cutânea in vitro. Entretanto, em países como o Brasil, enfrentam-se questões alfandegárias e perda do material por perecibilidade, dificultando e até impedindo, a importação desses kits para utilização por parte das indústrias e laboratórios nacionais. Em contrapartida, o desenvolvimento de um modelo de RHE apresenta-se como um avanço tecnológico e ganho de autonomia para esses países. Assim, no capítulo 1 explorou-se o desenvolvimento de um modelo nacional de RHE (USP-RHE) que atendesse às exigências internacionais descritas no guia OECD 439. O modelo desenvolvido apresentou uma epiderme bem diferenciada e atendeu aos parâmetros de qualidade (histologia, viabilidade e função barreira) bem como da funcionalidade, a qual é expressa na capacidade de distinção entre irritantes e não irritantes, apresentando 85,7% de especificidade, 100% sensibilidade e 92,3% de acurácia quando comparada com a classificação in vivo obtida pelo ensaio do linfonodo local (LLNA). No capítulo 2, células monocíticas THP-1 em monocamada foram capazes de distinguir entre agentes sensibilizantes e não sensibilizantes por meio da expressão de CD86, CD54 e liberação de IL-8. Após a obtenção de RHE e THP-1 funcionais, um cross-talking foi estabelecido gerando uma RHE imunocompetente. A RHEI distinguiu satisfatoriamente entre agentes sensibilizantes e não sensibilizantes por meio da expressão de CD86 e CD54 na membrana das células THP-1. A liberação de IL-8 também foi avaliada na RHEI, mas, não demonstrou ser um bom indicador para a avaliação de sensibilização, ao contrário de IL-1α, que distinguiu satisfatoriamente agentes sensibilizantes de não-sensibilizantes, mas não foi capaz de hierarquizá-los. No capítulo 3, avaliou-se o papel de interleucinas do tipo Th2 e da depleção de colesterol na membrana plasmática no desenvolvimento de características morfológicas e moleculares da dermatite atópica (DA) in vitro em um modelo de RHE. Os resultados demonstram que o uso de IL-4, IL-13 e IL-25 em combinação com a depleção de colesterol na membrana plasmática mimetiza in vitro, as principais características da DA. No capítulo 4, buscou-se avaliar os efeitos imunossupressores da radiação ultravioleta na RHEI. Os ensaios foram realizados em diferentes períodos de exposição, entretanto, não foi possível observar tais efeitos. Os resultados justificam-se pela ausência da liberação de IL-10 pelo RHE imunocompetente, por exemplo, e demonstram uma limitação do RHE imunocompetente para avaliações de inativação da reposta imune. Neste trabalho, concluímos que foi possível obter uma RHE competitiva, similar aos modelos internacionais validados e que pode ser utilizada como plataforma para ensaios de irritação e sensibilização cutânea, além de ser uma plataforma para estudos da dermatite atópica. No modelo é possível estudar a ativação do sistema imune, o que o torna promissor como uma plataforma para avaliação de resposta imunológica in vitro. Conclui-se, portanto, que os objetivos foram amplamente atendidos além de oferecermos um protocolo de livre acesso para reprodução por outros laboratórios e um modelo para validação futura


The development of new in vitro skin models and new methodologies follows a global trend in search for reductions or replacement of animal testing. In this context, Reconstructed Human Epidermis kits (RHE) are presented as a promising platform in the search for alternative methods to animal use, and some models are validated for skin irritation and corrosion in vitro tests. However, in countries such as Brazil, who face customs issues and loss of material due to perishability, making it challenging and even compromising the importation of these kits for use by industries and laboratories. In contrast, the development of an RHE model is presented as a technological breakthrough and gain of autonomy for these countries. Thus, in Chapter 1 we explored the development of a national model of RHE (USP-RHE) that meet international requirements described in OECD TG 439. The developed model presented a well-differentiated epidermis and met the quality parameters, for instance, histology, viability, and barrier function as well as the functionality expressed in the capacity of screening between irritants and nonirritants, with 85.7 % of specificity, 100 % of sensitivity and 91.7% of accuracy in comparision to in vivo UN GHS classification from Local limph node assay (LLNA). In chapter 2, monocytic THP-1 cell line, as monolayers, were able to distinguish between sensitizers and non-sensitizers by expression of CD86, CD54, and IL-8 release. In this model, functional RHE and THP-1 were used in a cross-talking, and thus an immunocompetent RHE (RHEI) was generated. The RHEI has distinguished satisfactorily between sensitizers and non-sensitizers through CD86 and CD54 expression that was larger and more sensitive in this model. The release of IL-8 was also evaluated in RHEI, however, did not demonstrate to be a good parameter for this evaluation, unlike IL-1α, which satisfactorily distinguished sensitizers from non-sensitizers, but was not able to hierarchize them. In chapter 3, we evaluated the role of Th2-related cytokines and plasma membrane cholesterol depletion (CD) in the development of atopic dermatitis (AD) morphological and molecular characteristics in an in vitro model of RHE. The results showed that combination of IL-4, IL-13 and IL-25 in combination with CD can reproduce the major features of AD in vitro. In Chapter 4, we sought to evaluate the ultraviolet radiation-induced immunosuppressive effects in RHE. The tests were performed at different times. However, it was not possible to observe such effects. The results are justified by the absence of IL-10 release by RHEI, for example, and show a limitation of RHEI for rating inactivation of the immune response. In this work, we conclude that it was possible to obtain a competitive RHE similar to the validated international models that can be used as a platform for irritation and skin sensitization tests, besides being a platform for the study of atopic dermatitis. Using this model is possible to explore the activation of immune system, which makes it promising as a platform for the evaluation of immune response in vitro. We conclude, therefore, that the objectives have been met as well as it is offering an open source protocol for breeding by other laboratories, thus offering the RHE model developed here for future validation tests

Antecedentes: Las dermatosis son patologías que se presentan frecuentemente en poblacio- nes de países en vías de desarrollo. Las enfer- medades de la piel constituyen uno de los prin- cipales motivos de consulta externa general, representan del 6 al 24% de la consulta pediá- trica. Objetivo: Caracterizar epidemiológica y clínicamente las cinco dermatosis frecuentes en consulta dermatológica pediátrica del Insti- tuto Hondureño de Seguridad Social, Región Noroccidental (IHSS-HRN) en el período de mayo a septiembre de 2015. Pacientes y Métodos: Estudio cuantitativo, descriptivo, transversal, realizado en niños de 0 meses a 11 años que reunieron los criterios para participar en el estudio de las cinco dermatosis más frecuentes. Los datos se recolectaron mediante una encuesta aplicada al responsable o tutor del niño participante. Resultados: Las prime- ras 5 dermatosis en frecuencia fueron: Dermati- tis atópica 133 (40%), prurigo por insectos 58(18%), molusco contagioso 56 (17%), urtica- ria 44 (13%) impétigo 38 (12%), Los 329 pacien- tes evaluados provenían del casco urbano. Todos pertenecían a la etnia mestiza, a excep- ción de un afro descendiente. El método diag- nóstico fue clínico en el 100% de los casos. ABSTRACT

A dermatite atópica (DA) é a doença dermatológica crônica mais frequente na infância e traz impacto negativo na qualidade de vida do paciente e de seus familiares. Os probióticos surgem como uma nova opção terapêutica, pela sua capacidade de modulação da reposta imunológica do hospedeiro, produzindo benefícios à saúde. Realizou-se uma busca nas plataformas de dados científicos PubMed, SciELO e Cochrane, apurando estudos nos idiomas português, inglês e espanhol. Baseado nos bancos de descritores DeCS e MeSH foram utilizadas as seguintes palavras/expressões: probiotics, probiotics and atopic dermatitis, atopic dermatitis and child e suas correspondentes nas demais línguas, sendo selecionados estudos entre 1993 e 2014. Considerando o papel dos probióticos na prevenção da DA, foram encontrados sete ensaios clínicos randomizados, sendo que quatro destes apresentaram desfecho positivo e os demais foram inconclusivos. Avaliando-se as possibilidades terapêuticas dos probióticos, cinco dos sete estudos analisados evidenciaram que a imunomodulação exercida pelas cepas probióticas tem melhor resposta à DA, além de menos efeito colateral que o obtido com o uso de corticoides e imunossupressores sistêmicos. O uso de probióticos na prevenção e no tratamento da DA mostrou-se seguro e eficaz, com descrição de efeitos colaterais em apenas um estudo. É possível que a suplementação com probióticos alcance seu lugar no manejo clínico da DA, podendo ser uma alternativa mais interessante e menos onerosa se comparada à terapia conservadora utilizada atualmente.


Atopic dermatitis (AD) is the most common chronic skin disease in childhood and it causes negative effect on patients? and their families? quality of life. The probiotics arise as a new therapeutic option for their ability to modulate the host?s immune response and produce benefits on their health. The authors searched on the scientific databases PubMed, SciELO and Cochrane, assessing studies in Portuguese, English and Spanish. Based on the descriptive databases of MeSH and DeCS, the following words/ phrases were used: probiotics, probiotics and atopic dermatitis, atopic dermatitis and child and their correspondents in other languages, studies were selected between 1993 and 2014. Considering the role of probiotics in the prevention of AD, seven randomized controlledtrials were found, four of those had a positive outcome and the others were inconclusive. Evaluating probiotics? therapeutic potential, five of the seven analyzed studies showed that the immunomodulation exerted by probiotic strains has a better answer for AD, as well as minor side effects than those obtained with the use of systemic corticosteroids and immunosuppressants. The use of probiotics in the prevention and treatment of AD proved to be safe and effective, being that side effects have been reported in only one study. It is possible that supplementation with probiotics can play a role in the clinical management of AD, being a more interesting and less expensive alternative compared to conservative therapy currently used

La denominada "marcha alérgica" se caracteriza por diferentes manifestaciones atópicas relacionadas y sucesivas a lo largo de la vida del individuo. Los niños que presentan alergia alimentaria (AA) tienen mayor predisposición al desarrollo de otras enfermedades alérgicas entre las cuales destacan dermatitis atópica (DA), asma y rinitis alérgica. La DA y AA coexisten en mayor medida en los pacientes que presentan DA de comienzo precoz, agresiva y persistente. Por su parte, la AlA es un factor precipitante de DA en un subgrupo de pacientes especialmente aquellos con AA mediadas por IgE, y también existiría correlación con las manifestaciones de AA de tipo retardadas. La disfunción en la barrera epitelial principalmente atribuida a mutaciones en el gen de la filagrina se ha descrito como posible desencadenante de la sensibilización a alérgenos por aumento de la permeabilidad cutánea. Se describen las características generales de la DA y evidencias de investigaciones actuales con respecto al rol de la AA sobre el desarrollo de la DA, su manejo y estrategias de prevención. Se discute la utilidad de los exámenes para el diagnóstico y las indicaciones de tratamiento y prevención en el manejo de niños con DA y AA. La restauración de las alteraciones de la barrera cutánea para prevenir la sensibilización antigénica tendría un rol importante para evitar el desarrollo de enfermedades alérgicas especialmente respiratorias.


The term "allergic march" refers to the history of different atopic manifestations throughout the patient's life. Children with food allergy (FA) are more predisposed to the development of other allergic diseases such as atopic dermatitis (AD), asthma and allergic rhinitis. AlD and FA coexist to a greater extent in patients with early signs of AD, aggressive and persistent symptoms. Meanwhile, FA is a precipitating factor to AlD especially in patients with IgE-mediated FA. Correlation to delayed manifestations of FA may also be found. Epithelial barrier dysfunction, mainly attributed to mutations in the filaggrin gene, has been described as a possible trigger for allergen sensitization by increasing skin permeability. This study describes general characteristics of DA and current research evidence regarding the role of FA in the DA development, management and prevention strategies. Also, the utility of diagnostic tests, treatment and prevention in children with DA and FA are discussed. The restoration of impaired skin barrier to prevent sensitization to antigens may have an important role to prevent the development of allergic diseases, especially respiratory diseases.

ABSTRACT

In humans, skin barrier dysfunction is thought to be responsible for enhanced penetration of allergens. Similar to conditions seen in humans, canine atopic dermatitis (CAD) is characterized by derangement of corneocytes and disorganization of intercellular lipids in the stratum corenum (SC) with decreased ceramide levels. This study was designed to evaluate the effects of a moisturizer containing ceramide on dogs with CAD. Dogs (n = 20, 3~8 years old) with mild to moderate clinical signs were recruited and applied a moisturizer containing ceramide for 4 weeks. Transepidermal water loss (TEWL), skin hydration, pruritus index for canine atopic dermatitis (PICAD) scores, and canine atopic dermatitis extent and severity index (CADESI) scores of all dogs were evaluated. Skin samples from five dogs were also examined with transmission electron microscopy (TEM) using ruthenium tetroxide. TEWL, PICAD, and CADESI values decreased (p < 0.05) and skin hydration increased dramatically over time (p < 0.05). Electron micrographs showed that the skin barrier of all five dogs was partially restored (p < 0.05). In conclusion, these results demonstrated that moisturizer containing ceramide was effective for treating skin barrier dysfunction and CAD symptoms.

INTRODUCTION: Ascaris lumbricoides-infected patients present lower prevalence of severe atopic dermatitis. METHODS: Peripheral blood of infected children with atopic dermatitis was assessed by flow cytometry of the frequency of Th1 and Th2 cells through the expression of CXCR3 and CCR4 chemokine receptors, respectively. RESULTS: Helminth-free patients with atopic dermatitis presented a high frequency of CCR4+Th2 cells. Parasitized patients with atopic dermatitis showed a lower frequency of CXCR3+Th1 cells compared to infected individuals only. CONCLUSIONS: Ascariasis modifies the blood traffic of Th2 cells in atopic dermatitis patients, while the allergic disease down-regulates the traffic of Th1 cells in parasitized patients.


INTRODUÇÃO: Pacientes infectados com Ascaris lumbricoides apresentam menor prevalência de dermatite atópica grave. MÉTODOS: Sangue periférico de crianças infectadas com dermatite atópica foi analisado por citometria de fluxo quanto à frequência de células Th1 e Th2 pela expressão de receptores de quimiocina CXCR3 e CCR4, respectivamente. RESULTADOS: Pacientes sem helmintos com dermatite atópica apresentaram alta frequência de células Th2CCR4+. Pacientes parasitados com dermatite atópica apresentaram menor frequência de células Th1CXCR3+ comparados aos indivíduos apenas infectados. CONCLUSÕES: Ascaridiases altera o tráfego sanguíneo de células Th2 em pacientes com dermatite atópica, enquanto a doença alérgica diminui o tráfego de células Th1 em pacientes parasitados.

FUNDAMENTOS: É bastante divulgado, sem a fundamentação científica necessária, que a infecção por molusco contagioso tende a ser mais frequente e de maior intensidade nos pacientes acometidos por dermatite atópica. Tal fato motivou a realização deste trabalho. OBJETIVOS: Avaliar a prevalência de associação de dermatite atópica e molusco contagioso; avaliar se, nos pacientes com dermatite atópica, a infecção por molusco contagioso é mais recorrente e/ou disseminada e se a ocorrência de eczema perimolusco, prurido e/ou infecção é mais prevalente que nos pacientes sem dermatite atópica. MÉTODOS: Avaliaram-se 284 crianças de ambos os sexos, com diagnóstico de molusco contagioso e/ ou dermatite atópica. RESULTADOS: Apenas 13,4 por cento dos pacientes apresentavam ambas as doenças. O número de áreas anatômicas afetadas por molusco contagioso, a ocorrência de eczema perimolusco e prurido foram maiores nos pacientes com dermatite atópica associada. Não houve diferença significativa de frequência de infecção secundária associada às lesões de molusco contagioso nos pacientes com e sem dermatite atópica. CONCLUSÃO: A prevalência da ocorrência de dermatite atópica e molusco contagioso associados foi baixa. Não houve diferença significativa na recorrência da infecção por molusco contagioso e na quantidade de lesões nos pacientes com e sem dermatite atópica. O número de áreas anatômicas afetadas por lesões de molusco contagioso, a presença de eczema perimolusco e de prurido foram maiores nos pacientes com dermatite atópica. Não houve diferença significativa na ocorrência de infecção bacteriana secundária nos grupos com e sem dermatite atópica.


BACKGROUND: Although no scientific evidence has yet been published, it is widely understood that molluscum contagiosum tends to be more common and more intense in patients with atopic dermatitis. This lack of evidence led to the development of the present study. OBJECTIVES: To evaluate the prevalence of the association between atopic dermatitis and molluscum contagiosum; to evaluate whether molluscum contagiosum is more likely to be recurrent and/or disseminated in patients with atopic dermatitis and whether the occurrence of eczema surrounding the molluscum contagiosum lesions, pruritus and/or infection is more prevalent in these patients compared to patients without atopic dermatitis. METHODS: A total of 284 children of both sexes with a diagnosis of molluscum contagiosum and/or atopic dermatitis were evaluated. RESULTS: Only 13.4 percent of the patients had both conditions. The number of anatomical areas affected by molluscum contagiosum and the occurrence of surrounding eczema and pruritus was greater in the patients who also had atopic dermatitis. There was no statistically significant difference in the frequency of secondary infection associated with the molluscum contagiosum lesions between the patients who had atopic dermatitis and those who did not. CONCLUSION: The prevalence of atopic dermatitis associated with molluscum contagiosum was low. There was no statistically significant difference in the recurrence rates associated with molluscum contagiosum or in the number of lesions between the patients who had atopic dermatitis and those who did not. The number of anatomical areas affected by the molluscum contagiosum lesions and the presence of surrounding eczema and pruritus were higher in the patients with atopic dermatitis. There was no statistically significant difference in the occurrence of secondary bacterial infection between the groups with and without atopic dermatitis.

ABSTRACT

Cow's milk is one of the most common food allergens in children with atopic dermatitis (AD). This study was conducted to describe the natural course of cow's milk allergy in children with AD, and to identify factors predictive of outcome. To accomplish this, we reviewed the medical records of 115 children who were diagnosed with AD and cow's milk allergy before 24 months of age to evaluate their clinical characteristics and prognostic factors. During a follow-up period of 24 to114 months, the median age for tolerance to cow's milk was found to be 67 months. Multivariate analysis using the Cox proportional hazard model revealed that the peak cow's milk-specific IgE level within 24 months after birth was the most important factor for prediction of the outcome of cow's milk allergy. In conclusion, half of the children younger than 24 months of age with AD and cow's milk allergy could tolerate cow's milk at 67 months of age. The peak cow's milk-specific IgE level within the first 24 months of birth is useful to predict the prognosis of cow's milk allergy in children with AD.

ABSTRACT

The point prevalence of atopic dermatitis (AD) among Korean adults visiting the Health Service Center for health check-ups was assessed. AD was diagnosed based on the questionnaires filled by patients and through physical examination by dermatologists. A total of 3,563 persons completed the questionnaires, and 2,032 persons were examined by the dermatologists. According to the questionnaires, the prevalence of AD was 7.1%, and from the dermatologists' examination, it was 2.6% overall, which decreased with age significantly. The prevalence in men was higher than that of women. Grading the severity of AD according to their eczema area and severity index scores, 70.6% were classified as being mild, 25.5% moderate, and 3.9% severe. Interestingly, concomitance of psoriasis and AD was found in 0.5% of those examined by the dermatologists. Our results suggest that AD is one of the most common skin diseases not only in children but also in adults in Korea.

OBJETIVO: Verificar os fatores de risco para sibilância em lactentes no sul do Brasil. MÉTODOS: Estudo transversal, onde foram aplicados questionários padronizados e validados (Estudio Internacional de Sibilancias en Lactantes - EISL), aos pais de lactentes com idade entre 12 e 15 meses que procuraram 35 das 107 unidades de saúde para imunização rotineira no período entre agosto de 2005 e dezembro de 2006. Foi realizada análise univariada entre sibilância e os fatores estudados utilizando razão de prevalência (RP) e intervalo de confiança de 95 por cento (IC95 por cento). Fatores associados à sibilância na análise bivariada foram avaliados com a utilização de regressão de Poisson. RESULTADOS: Três mil e três lactentes participaram do estudo. Os fatores de risco para sibilância foram: gênero masculino (RP = 1,14; IC95 por cento 1,05-1,24), história familiar de asma [mãe (RP = 1,18; IC95 por cento 1,04-1,33); pai (RP = 1,20; IC95 por cento 1,05-1,39); irmãos (RP = 1,23; IC95 por cento 1,08-1,42)], outros animais domésticos (pássaros, coelhos, etc.) presentes durante a gravidez (RP = 1,28; IC95 por cento 1,07-1,53), idade de início na creche [0-3 meses (RP = 1,15; IC95 por cento 0,98-1,34); 4-6 meses (RP = 1,39; IC95 por cento 1,24-1,55); 7-12 meses (RP = 1,20; IC95 por cento 1,07-1,35)], seis ou mais episódios de resfriado (RP = 1,32; IC95 por cento 1,21-1,44), história pessoal de dermatite (RP = 1,09; IC95 por cento 1,003-1,19) e mofo no domicílio (RP = 1,14; IC95 por cento 1,04-1,24). Imunização atualizada (RP = 0,79; IC95 por cento 0,63-0,98) e banheiro no domicílio (RP = 0,83; IC95 por cento 0,68-1,01) foram fatores de proteção. CONCLUSÃO: Os fatores de risco independentes para sibilância no primeiro ano de vida também são conhecidos como risco para asma em crianças e adolescentes. Esses dados são úteis para prever o diagnóstico de asma e instituição de medidas de prevenção quando cabíveis.


OBJECTIVE: To assess risk factors for wheezing in infants in southern Brazil. METHODS: Cross-sectional study using a standardized and validated questionnaire (Estudio Internacional de Sibilancias en Lactantes - EISL, or International Study of Wheezing in Infants). Parents of infants aged 12-15 months who attended 35 of 107 health centers between August 2005 and December 2006 for regular immunization were interviewed. The association between wheezing and factors studied was made using a prevalence ratio (PR) and confidence interval of 95 percent (95 percentCI) to perform a univariate analysis. Factors associated with wheezing in the bivariate analysis were studied using Poisson regression. RESULTS: Three thousand and three parents of infants filled out the questionnaire. The risk factors were male gender (PR = 1.14; 95 percentCI 1.05-1.24), history of asthma in the family [mother (PR = 1.18; 95 percentCI 1.04-1.33); father (PR = 1.20; 95 percentCI 1.05-1.39); siblings (PR = 1.23; 95 percentCI 1.08-1.42)], other pets in the home during pregnancy (PR = 1.28; 95 percentCI 1.07-1.53), age when child started daycare [0-3 months (PR = 1.15; 95 percentCI 0.98-1.34); 4-6 months (PR = 1.39; 95 percentCI 1.24-1.55); 7-12 months (PR = 1.20; 95 percentCI 1.07-1.35)], six or more episodes of cold (PR = 1.32; 95 percentCI 1.21-1.44), personal history of dermatitis (PR = 1.09; 95 percentCI 1.003-1.19), and mold in the home (PR = 1.14; 95 percentCI 1.04-1.24). Up-to-date immunization (PR = 0.79; 95 percentCI 0.63-0.98) and bathroom in the home (PR = 0.83; 95 percentCI 0.68-1.01) were protective factors. CONCLUSIONS: Independent risk factors for wheezing in the first year of life are also known risks for asthma in children and adolescents. These data are useful to predict the diagnosis of asthma and to promote its prevention (when applicable).
